This is a professional intake assessment. Its purpose is to evaluate your dog's situation and determine the right path forward. If we are a fit, next steps and recommendations will follow from there.

Most people come to Revive & Thrive because something is not right with their dog and they have run out of conventional answers. This assessment is where we start. Before any plan is built, we need to understand the full picture: health history, current concerns, what has been tried, and what you are actually trying to achieve.
You will leave this call with clarity on whether integrative wellness is the right fit for your dog, and what working together would look like.

How to Book

Complete both steps to secure your assessment. Your spot is not held until both are done.
Step One
Complete the Pre-Call Questionnaire
Before we meet, I need to know your dog's background. This form covers health history, current concerns, lifestyle, and your goals. It takes about 10 to 15 minutes and allows us to make full use of our 45 minutes together.
Do not skip ahead to booking until this form is submitted. Your answers shape the entire call.

Schedule and Pay
Once your questionnaire is submitted, book your assessment directly through the calendar. Payment is collected at the time of booking via Stripe. Your appointment is confirmed once payment is complete. You will receive a confirmation from Google Calendar with your video call link.

To reschedule, contact us at least 24 hours before your appointment. One reschedule is permitted per booking at no additional charge. Subsequent reschedule requests may be subject to a rescheduling fee.
